Elective Request Forms and Applications

Elective Request Forms for current 6th and 7th grade students are due to your Advisory teachers by Friday, April 13th. If your requested electives require an application, attach it to your Elective Request Form.

Independent Study Physical Education (ISPE)

ISPE qualifications include (see application packet for a more detailed explanation):

  • Participant must compete in an individual sport at a regional, state, or national level
  • Proof of participation in at least 3 competitions at that level in the last 6 months
  • Participant must spend a minimum of 200 minutes per week in the ISPE sport


2018-2019 ISPE Application Packet is due to the CHMS front office by June 1, 2018.

Math and English Placement Information

Letters have been sent home about your child's Math and English placement for next year based on the above criteria.


If you would like to submit a waiver, please do so by June 1st.
