California Healthy Kids - Parents Survey

Dear CHMS Parents and Guardians,

Each year, we administer the California Healthy Kids Survey, sponsored by the California Department of Education, to 7th grade students and all CUSD parents and staff. This is a very important survey that will help promote better health among our youth and combat problems such as drug abuse and violence. Please complete this online survey as it is an opportunity to communicate your perceptions about CHMS. Your participation is voluntary. It is also very important. The information you provide will help guide district and school efforts to promote safety, enhance learning supports, and improve student achievement. You can access the survey here. If you have any questions about it, please contact Rosemary Eshelman, Student Services Specialist at Here are the answers to some FAQ’s about the survey:

This is an anonymous survey. It is designed so that no one can be identified from the data. You do not have to respond to any questions that you feel could be used to identify you. Simply skip any such questions.

All questions apply to CHMS only.

The results are for the use of our district and school. The California Department of Education will preserve data confidentiality and refer any data requests to the district.

If you have more than one child at this school, you will only need to complete one survey for this school. Please fill out just one survey about the school using your oldest child as a reference.

For access to the survey, click this link: CHMS California Healthy Kids Survey or go to

Thank you for your participation,

Corey Espeleta

Attendance Procedures

We have been receiving several inquires about the proper procedures to call your student out of school early. If your student has a doctor appointment or personal appointment that makes it necessary for him to leave school, please send a note to or call the Attendance Office in the morning so we can issue an off-campus pass. Please do NOT bring the note to your teacher, it must go directly to the attendance office.

  • This enables the school to account for the student’s whereabouts.
  • When a student has obtained a pass, the Attendance Office will clear the periods he is gone.
  • Your student may be waiting in the office for you to pick up, saving you time.
  • The school will receive full ADA funding for your student’s partial day attendance.

Include in the note what time your student needs to leave and the reason he is leaving (doctor or dentist appointments, personal reasons, etc.)

If  you are calling in an absence, please call our 24-hour ATTENDANCE HOTLINE at 760-331-6470. For any other questions about attendance, please call Marilyn Bergsma at 760-331-6464 or visit our Attendance page.
